Required Documentation
Needed for Event Day Check In:
- Copy of Official State Roster
All teams MUST have updated roster in GotSport tournament event prior to check in deadline. Official rosters will be printed on game cards and only players on the game card will be allowed to dress out.
- Official State Travel Permit
All out of the state of Arkansas teams must present this document at check in. (contact your club registrar or state association for travel permit) US Club teams not included.
- ORIGINAL State Official Laminated Photo Player Card
PER Players/Coaches/Team Manager for the current season.
- Club Pass
Accepted and recommended if players are from same club.
*If your club does not utilize club pass, GUEST PLAYERS must be added to your GotSport team roster under the tournament event. All GUEST PLAYERS must have Official Guest Player Forms signed and Official Player Card. Guest player limit of 3.
- Birth Certificates
Birth Certificates are not required for check-in, but must be available during tournament if asked to show player proof of age.
- Tournament Medical Waiver Document
Every team must sign the Tournament Medical Waiver Document to verify the team has medical waivers for all players. Only 1 form per team is needed; please do not send all player medical waivers.
